Parent E-Signature Forms

UtilityLightweight e-signing for school athletics forms: parents view a PDF in the browser, sign on a canvas and submit. The server stamps a signature certificate page onto the PDF and emails copies to the parent and the athletics office. Deliberately lightweight, not ESIGN/UETA-grade.
